We start by treating you as a unique individual. We’ll create a plan that addresses your dental care needs, and we can provide a full range of services, from routine check-ups to digital X-rays and any other treatments you may need. We even have our own dental lab, which lets us save you time and money.
New Patients
We’re looking forward to meeting you. If you have not completed the new patient form below, please arrive 15 minutes before your appointment. Also, please bring with you:
- Photo ID (required)
- Dental insurance card, if you are insured
- Patients under 18 must be accompanied by a legal guardian
- Patients over 18 with a legal guardian must be accompanied by their guardian
Insurance
